For Medicare Part A reimbursement, there is no regulation that requires specific charting from specific disciplines.  The purpose of documentation for Part A is to demonstrate that the resident received medically necessary daily skilled services that the facility is billing for.  The facility can determine for itself how it will demonstrate that.

As a standard of clinical practice, it is also necessary to document the resident's response to care and services.

Is there a requirement that there should be a Medicare note on every resident covered? Dietary, Recreation, Rehab and Social Work does their notes for initial, 14 day, 30 day, 60 day and 90 day schedule.  What is Nursing's responsibility with regards to nurses note? Thank you.
