I know that 14 on the 802 "Unintended Weight Change/Nutrition/Swallow/Dentures" is one place where you can check if the resident is in a restorative dining program, however that should pull from the MDS
 
Per the Roster/Sample Matrix Provider Instructions " If P3h is greater then 0, then 802-14 = R"
 
In addition 28 on the 802 "Assistive Devices" is checked if P3c(Splint or brace assistance) is greater then 0 from the Roster/Sample Matrix provider instructions.
 
Don't know of any that you would need to code manually

Basically I was asking about manually inputting information on the 672/802 forms. some soft wares automatically pull your MDS information forward on to these forms however some areas do not & they have to be input manually. So all I was wondering was does anyone know where does Restorative Services by nursing coded on the 672/802.
Is it under Specialized Rehab?
Skilled Rehab?- These reports are given to surveyors upon entry. They have to be up to date to the current date on all residents. There fore I plug information in daily to maintain current information on our 672/802 form.

I'll try to help but, sorry to be dense, I'm not totally sure what you're asking.  There are F tags that address the Quality of Care issue -
and the MDS manual  at Chapter 3: 191-197 is your source for the criteria for the Restorative Programs.  You'll need to document to support the programs as per the instructions in the manual.
You'll mark the qualifying programs on the MDS at P3.
Restorative Programs are intregal to Rehab Low, Cog. Impaired, Behavior, and Reduced Physical Function RUG categories.

Under Skilled nursing services? 
Part of the Definition under the General instructions for  672  states that Maitenance programs have to be developed by a Therapist but  what about follow up RA programs after they discharge and teach RA personell. Would this still count under Skilled nursing services?
Right now RN follows up q month to assess how residents in RA are progressing, DC orders accordingly or continue.
