We found forms don't work. Once a year, at
one of our monthly unit meetings, we had each staff member a piece of note paper
and ask them to write down at least one thing, they would like more
education/inservicing on.

This is not exactly related to MDS; but does
anyone have a survey or questionnaire they use to determine facility's
education needs and wants. It seems like we all give lots of in-service
programs based on what we think staff needs, but does anyone have a
structuring for asking {all} staff what they would like additional education
or resources in?
