Staff Education

This is not exactly related to MDS; but does anyone have a survey or questionnaire they use to determine facility's education needs and wants. It seems like we all give lots of in-service programs based on what we think staff needs, but does anyone have a structuring for asking {all} staff what they would like additional education or resources in?   Thanks
