An OMRA is not done if you are placing a resident back on rehab services.  The only time an OMRA is done is when therapy services are discontinued and the resident still meets the guidelines to remain skilled under the med a program. 

 

The significant change is your best bet is the resident has had a decline which would indicate this assessment.

As a side note:

 

An OMRA is also done if you are placing a resident back onto rehab services.  The other stipulation is to do a Significant change as well if there was a decline in function from the initial discharge of rehab services.
