If there is documentation that she does answer
or you personally have noted her to understand and answer questions then code it
as it is....HOWEVER, make sure an accurate and updated care plan states the
inconsistency staff have noted, perhaps by using the words....selective to
answering questions depending upon her mood level of the day.

Subject: resident who rarely talks- how
do you code "ability to make self understood"?
We have a resident who rarely will answer questions or talk- she will
usually look at staff & smile only.... but, during the ref. period she did
answer questions such as "how do you feel" (stated, "I feel fine."), where is
your husband (her roommate- she stated "He is not in here right now.").
Her husband states she will tell him if she feels "good" or "bad". Basically,
when she does talk, she is able to make herself understood. How would you code
this?
