The resident has a right to self-administer medications per the federal guidelines if he/she has been deemed capable of doing so by the interdisciplinary care plan team. The way the resident is determined to be capable is through an assessment involving the resident being able to tell what meds they are on, dosages, etc. Briggs has an excellent self-administration assessment that easily assesses whether the resident is truly capable of self-administration. If the resident is capable of self-administration, then the facility has to follow their policy for storage of the medications at the bedside. Our facility has lock boxes that are made into the residents bedside table, but you can buy a lock box from walmart, etc. Then the resident is supplied with a key as well as the nurse responsible for this resident's meds, and the DON. The nurse responsible for that resident's medication administration every shift must check with the resident to ensure that he/she has taken the meds and then sign off such on the MAR (per our policy).
You must care plan self-administration of meds. I have attached a sample care plan for self-administration. Also, ensure that your facility has a policy for self-administration that matches what your staff are doing and what you are careplanning. Some facilities do have policies that do not permit self-administration of meds. I have never worked in one that did, but I do know that some facilities use this policy. Brenda W.
