I don't print a new MDS when I complete a correction form. I create a correction form and print it out, signing AT5 and dating AT6 and signing AT7 and dating it. I attach the correction form to the existing MDS, correct the existing MDS with the routine medical records correction procedure of drawing a line through the wrong answer, entering the correct answer and dating and signing the correction. I place a note on the front of the existing MDS noting what I corrected and when I discovered the error. I correct the MDS in the computer and retransmit.
