We do not do this at our facility.  I keep 2 binders for Medicare. I use a tracking log for each Med A resident with the days plotted out; there is space for information regarding the reason for skilled coverage, any comments, etc. When a resident is discharged from Med A, I write a note on the tracking log explaining why, and whether the resident remains at skilled level of care, how many days are remaining in the benefit period (if any)  I have one binder for current census and another for discharged.

When a resident comes off of MEDICARE PPS do you need to write an order discharging resident off of PPS?
 
There was some confusion with one response that I received on this in regards that it was being interpreted that I was talking about MEdicaid skilled.  This is in regards to PPS.
