Subject: Pressure sore vs. ischemic ulcer

Hello everyone,
       I have a patient admitted 2 mos. ago with stage 2 (scab ) on right heel.  It has healed after a few weeks then  recurred again.  She is ambulatory.  We treated it, healed, then recurred again.  Finally, we referred her for a vascular consult coz she's been complaining of pain too on that leg even just by touching it.  Vascular MD wrote that it is an ischemic ulcer and they did an arteriogram.  My question is, on our skin sheet, we first documented it as pressure sore, should we change our skin sheet now and consider it as non-pressure ischemic ulcer?  The thing that is confusing is that, it is on pressure point but it is not a pressure sore but ischemic ulcer.  Thank you so much for all your help.
