I wouldn't change it now, unless you had that diagnosis by the MD then. I would just note it now that the MD diagnosed as such, and code it that was on your next assessment.

My question is, on our skin sheet, we first documented it as pressure sore, should we change our skin sheet now and consider it as non-pressure ischemic ulcer? The thing that is confusing is that, it is on pressure point but it is not a pressure sore but ischemic ulcer.
