Did you say you don't attend careplan.I am now curious as to how many MDS coordinators 
don't actually run the careplan meeting.To me it all fits together.I work at a 
facility where I do all the nsg mds items on the PPS patients and take care of 
anything else that pertains to med A but have a partner,who does the nsg MDS items for 
the rest of the house.She writes the careplans along with the other disciplines for 
all residents,PPS included.She attends and facilitates the careplan 
meetings.Occasionally,she will point out a needed sig change.I must say others may ask 
my opinion as I am the senior coordinator but the final decision is ours.One member 
needs the full authority for sig change decisions,of course with input from other 
members of the team.During survey who do you think will be asked about why a sig 
change wasn't done.My guess would be the RN coordinator.After attending one of the 
AANAC programs,I believe Jan Stewart,suggested we have some kind of a form explaining 
sig changes etc.We use a form in our MDS section where we wr
ite notes regarding hospital stays and the rationale for doing sig changes or 
not.During survey,I am still nervous when they come and I have been in longterm over 
30 years,it is easy if you have notes when a question arises.
