MEDICARE DOCUMENTATION..WEEKLY OR DAILY??

What is Medicare's guideline for documentation..is it weekly or daily... ? Some facilities told me its daily and some just do weekly.  This documentation refers to skills that the resident has in order to be covered for Medicare. Thanks in advance.
