I'll add my 2 cents... I can't see how to justify a daily skilled service unless there is a daily note to support it. Our facility is grappling with once a day or once a shift for PPS. Although they complain about charting, I am still seeing charting pretty much on each shift for PPS admissions. We are trying to develop an easy form for daily charting, for PPS, antibiotics, IV, post fall and other incidentals that may require daily charting for a limited period of time regardless of payer.
