I’ve put Neosporin on many times without a dressing.  I would err on the side of caution and not code the dressing unless I knew for sure and had documentation that a dressing was done.

Ok,  I think I'm arguing semantics here, but here's the question.  A Medicare A resident has an order that reads  "apply neosporine to scratches on lower leg BID".  So my question is, can I "assume" that the neosporine was covered by a dressing?  or should I err on the side of caution and leave M5g blank?
