Actual the snf manual mentions remaining at a skilled level of care.It doesn't say 
gtubes.Remaining at a skilled level of care can be in other circumstances also.Had a 
diabetic,who had a fx hip and multiple other medical issues.Developed stage 4 decub on 
heel,stayed 100 days on med A between about 4 hospitilizations.Just went to surgeon 
and had a flap done.She has remained in our facility through out all this.We denied 
her med A again because of her stage 4, she never had a 60 day spell of wellness.Have 
had a few very unstable insulin diabetics who have used 100 days,went into hospital 
again but again we felt we couldn't find a 60 day spell of wellness.What I have done 
when a patient has used 100 days[of course this happens very seldom]I look back into 
their records monthly and let the business office know when they can start counting 
the 60 days.
