It does not matter about the ICF Medicaid bed.  If the resident continued to consume the skilled level of tube feedings, ie K6a is 51% or more calories or K6a is 26%-50% calories and K6b is 501cc or more per day of fluid enteral intake during the 60 day break, then the resident continued to be at a skilled level of care by medicare standards.

If a resident silled Med A for 100 days for tube feeding, then returned to an ICF medicaid bed for over 60 days (same tube feeding), went to the hopsital and returned with dx of pneumonia and requires therapy, can that resident be skilled Med A again (same tube feeding) for therapy and dx of pneumonia.
