It would depend on all the circumstances.  IF documentation supports that special arrangements were made to accommodate needs for a short, “special” occasion then we just call it a home visit.  You just can’t bill Medicare for any midnight they were not in the facility.  The bad part is that unless they have another payment source for those days (e.g. Medicaid) they have to pay privately so unless they were advised of their financial responsibility you get stuck taking the loss.  This info is now part of our admission agreement.
