In our facility, when the resident continues to fall, we have them evaluated by therapy to determine if they're still appropriate for their device (restraint..)It appears she is no longer safe since she can open the merri-walker. We would probably put her in a w/c with a lap tray/lap buddy (with recomm. from therapy) and have her walked with restorative. There is also a chair alarm that you may want to consider but many residents know how to shut the alarm.
