Have you considered her mental status?  It appears she is unable to remember
to ask for assist.  Would she perhaps be more appropriate in a walker that
she cannot open & walk out of?  Also, WHY is she getting out of the walker?
Does she need to be toileted, and maybe is not asking staff?  Does she need
a toileting program?   Is she getting out to sit in a more comfortable
chair?  Etc., etc.,
