I don't recall that anything in the Federal Register specifically about hospital-based SNFs.  I did ask several speakers at the AANAC Conference last year (Chicago) about this specifically because I was concerned about some residents getting 7 days and others only 5.  They indicated that we were simply providing services at the level required for each resident, and as long as it was at least 5 days, we were in compliance.  I would be very interested in other opinions, and especially source documentation.

There are still a lot SNFs that don't have Rehab services 7 days a week. Therapy will usually come in on a weekend day if needed to get enough minutes for the RUG catagory that is needed. Again, per the MDS crosswalk, resident has to have a certain number of minutes and @ least 5d days of rehab services to RUG out in a Rehab catagory. They can certainly have more days than the 5, but 5 is the LEAST number of days that qualifies.In my current facility, rehab usually only works 5 days a week, but still manage to get in enough minutes to RUG us in the HIGH, VERY HIGH, and even ULTRA HIGH catagories.

I have been told that if therapy is only offered 5 days a week, then that is considered daily.  I would think that if it is available 7 days and it is not given that it would not be considered daily to meet the Medicare requirements.

We are a hospital based SNF, Medicare only. Our Therapy Dept. which covers the entire hospital, our unit and out-patient rehab services has become short staffed. They are wanting to provide therapy 6 days a week, splitting the weekend. PT normally sees residents 7 day/week and OT 5-6days/week. Will this continue to meet Medicare guidelines as a daily service, even though therapy services are available 7 days/week, just not on our unit
