I would look at the current weight with the weight 30 days ago and 180 days ago for the K3a question.
But code the weight loss in Section J (loss of 3 or  more pounds in 7 day period).  And you have already
assessed and careplanned it.

Am seeking clarification on how you all would handle this---
A patient was admitted on 10/1 with an admission weight of 150(accurate).
On 10/7 the patient was weighed again--the weight was 136(accurate). An
assessment is done 10/8. There is a 10 lb weight loss, or 9.3%.
Does this get recorded at K3a? Or do we do as the RAI Manual says, and
compare the most recent weight only to a weight 30/180 days ago?
Weight loss has been care planned, and interventions in place.
