Template for RAP Module Forms

Does anyone have a template for RAP module forms already on their computer (like the Briggs forms, but don't want to infringe copyright) I prefer to do things on the computer, it is faster for me, and I like to save it since some things just don't change much.  If you have any you would be willing to share with me, I would appreciate it, thanks.
