I'm like you, I type much faster than I
write. I did my own set of RAPs for all the nursing stuff. I
basically just copied the stuff off the Keys, and then put in sections for the
nature of the problem, the complications and risk factors, and need for
referrals, if any and then the factors considered to develop the care plan.
I'm including a sample Vision RAP. Since I did them in Word, you will
have to either have Word on Word Perfect 7 or above on your computer to use
them. You will also have to remember to hit the INSERT key in the
word processor while you do them or else the format gets seriously off, and
items get pushed off the page.
If you can use it where you work, I can send you
the set of nursing RAPs. Just remember, I didn't do any for
Psychosocial, Mood or Behavioral RAPs.

Does anyone have a template for RAP module
forms already on their computer (like the Briggs forms, but don't want to
infringe copyright) I prefer to do things on the computer, it is faster for
me, and I like to save it since some things just don't change much. If
you have any you would be willing to share with me, I would appreciate it,
