Where can we get a copy of the denial notice?

At our facility we have the family sign a medicare denial sheet saying
they are waiving their medicare A benefit for hospice.
If a resident is on Medicare A and switches to hospice then we do the
same thing.  We have the family sign a Medicare denial notice.

We have a family that wants to bring their family member in as hospice instead of as Med A. We suggested using Medicare on our palliative care

unit and then switching to hospice and they are not interested.  The
hospital diagnosis is the same as the hospice diagnosis therefore he is
not
eligible for both at the same time.  Is there a form that needs to be
signed
waiving the medicare benefit?  Same question with a resident who wants
to
change to hospice in the middle of their Med A stay while they are still

skillable.
