The 700 forms are not mandatory so they do not have to be used; however, a
therapy plan of care and a certification by the physician for the necessity
of the services still needs to be in the record in some format that includes
the needed information.

We had a new rehab company here this weekend, and they said that the 700
forms are no longer required.

I am trying to locate "in writing" the requirement that we cannot bill
the FI until the certs & recerts, 700 & 701s are signed.

Can someone help me on this?
