You are not required to do a comprehensive assessment unless the resident has also experienced a significant change. (Manual pg 2-7 through 2-10). You need to do an OMRA using an ARD date of 8 to 10 days after all rehab therapies have been discontinued. The OMRA establishes a new non-therapy RUG category. (See Manual pg 2-31) If the resident has also had a significant change, you could combine the OMRA with that and you would have a comprehensive assessment that includes RAPs. (See also note on page 2-37 regarding combining assessments.) You keep using the rehab RUG score until the date of the OMRA.

When a MedA resident discharges from therapies but remains on skilled care (dialysis, IV meds., stage III decub., etc. do we do a comprehensive with RAPs to take them out of the rehab category, or can we keep the rehab RUG score? Thanks for your help.
