If this resident has never had an OBRA Admission assessment completed, then you cannot do a Significant Change.  You must code the initial assessment as an Admission.  In your RAPs, you can explain any changes that have occurred since admission.

I work in a skilled nursing unit and have a question about coding. We do all our 14 day MDS as a 1/7 We have a resident that has had a sig change (CVA) since admit can I code this as 3/7?  Will this still be an initial assessment? Thanks Lisa
