You would still code it as an admission assessment.  You cannot do a significant change before you do an admission assessment.

I work in a skilled nursing unit and have a question about coding. We do all our 14 day MDS as a 1/7 We have a resident that has had a sig change (CVA) since admit can I code this as 3/7?  Will this still be an initial assessment? Thanks Lisa
