I would ensure, though, that I could physically read this in the medical
record.  Having worked hospice, our paperwork did indicate that the
resident has 6 months or less to live (expected) but, at times here,
hospice does not always leave paperwork on the charts.

Does the Hospice admission paperwork include the terminal diagnosis or
that
she has less than six months to live? Usually, their paperwork includes
that
statement. We have a good relationship with our local Hospice and we can
always get copies of their admission paperwork when the patient signs up
with them. If not, I would call or fax and ask for the appropriate dx
and/or
statement before I checked it on the MDS.

Hi, Group,
 
I have a new admission, whose D/C Summary contains the following:  "Is
not
eating well and has a decline, but she is 94 and it is very hard to be
optimistic about her outlook."
 
Since admission, she was out to ER and was to be admitted, but resident
and
family stated they wanted no further intervention, and wanted to return
to
NH "so she could die."  Hospice consult has been made.
 
In this situation, even though the words, "less than 6 months to live"
haven't been written, would it be appropriate to code J5c?
