I would not code J5c unless I had a written statement by the MD addressing “less than 6 months to live.

Hi, Group,

 

I have a new admission, whose D/C Summary contains the following:  "Is not eating well and has a decline, but she is 94 and it is very hard to be optimistic about her outlook."

 

Since admission, she was out to ER and was to be admitted, but resident and family stated they wanted no further intervention, and wanted to return to NH "so she could die."  Hospice consult has been made.

 

In this situation, even though the words, "less than 6 months to live" haven't been written, would it be appropriate to code J5c?
