I think the major issue is not the frequency of times toileted during the day, nor the specific schedule that is used; the primary issue is that the plan is specific, measurable and DOCUMENTED.  If you do it on rising, before and after meals, HS and prn, and don't document, then it is not a scheduled toileting plan for MDS purposes.
