Which documentation do you use?????   And   is it appropriate for me to interview the staff  and write a clarification note   it would be dated  Jan 8th and I would reference the ARD of Jan 6th.

I have always written notes based on interviews with employees.  Remember, if the cna's document 3-2, and the nurses document 4-2, it is acceptable to take the highest level of care no matter who is delivering it.  I write the note to clarify the discrepancy.
