I would do the forms if she was readmitted to your facility for those hours. She would have been discharged from the hospital when she left them and arrived at your faciity with the intent of being admitted. Doing the reentry and then discharge tracking shows the movement and acuity of the resident.

Hi,
We have a resident who was sent to the hospital, stayed for a few days, returned to us, was only here a few hours, was sent back to the hospital and readmitted to the hospital for about a month. She came back to us yesterday. Do I need to do an reentry and discharge return anticipated for the day she came back from the hospital for a few hours and was then sent back?
