Coding in Section P

I have a question for the group:   In order to code section P of the MDS the RAI manual states therapy must be ordered by a physician: Must the physicians order be a valid order (valid meaning: signed and dated) before the information is coded on the MDS?   Thank you, Denise Pizzino
