Per the RAI manual, page 3-198:
"To record the frequency, over the last seven days, with which the resident was restrained by any of the devices listed below at any time during the day or night. The intent is to evaluate as part of the assessment process whether or not a device meets the definition of a physical restraint, and then to code only those devices categorized in section P4 that have the effect of restraining the resident."
 
The manual is very clear in reinforcing that the intent of the device is not so important as the effect of that device. In other words, even though a device may be intended to enable the resident (side rails, waist belt, etc), if it meets the definition of a restraint, then it must be coded in P4.  By the same token, some devices may have the effect of restraining, without fitting into the categories listed.  You would not code in P4, but you better have an order for the device, a medical reason for it, and a care plan.

Please share how you code Section P Devices and Restraints.  If siderails are for enablers only, do you mark siderails in that section at all?  Do you code devices that are NOT restraints (i.e. lap buddy which R can remove)?
