My first question would be what sort of a discharge
tracking did you do when they went to the hospital? I will guess that it
was an 8 - Discharge to completing initial assessment since you coded the
assessment when they returned an an Admission Assessment(AA8A=1), or were they
from a non certified unit and you didn't do assessments on this resident?.
I think your problem is driven from the fact that your AB1 date is 1/8/04 (Date
the stay began - Note - does not include readmission if record was closed at
time of temporary discharge to hospital etc. In such cases, use prior admission
date) and your date of reentry A4A date
=1/8/04. (So you have the same date for date of entry and date of
reentry)
So,
was this resident already in a Medicare/Medicaid certified bed when they left
for the hospital? Were you doing MDS assessments on him/her and
transmitting them? If Yes, I don't think your AB1 date would not be
1/8/04, which is why I ask about the tracking form. However, lets say this
resident was New to the Certified Unit when they returned and you are now
just beginning to do MDS assessments on them you would leave A4A
blank.
