Perhaps the lady had the fall due to dehydration,  possible delirium, etc. and there actually was no injury. Remember, Medicare A requires daily skilled services at 5 X's a week. Did you cover under presumption? Perhaps therapy will reconsider for 5 x's week or OT can pick up the other 2 days.      3 x's week will not cover under Med A unless you have 2 RNP's 6 x's a week.
