Therapy screens with Annuals

Can the group give me some suggestions for Therapy screening annuals. They are picking up the patient too late when an annual is comming up.  So we are doing an annual assessment and then maybe 2 weeks later a Sig Change.  I suggested they start screening them 2weeks b4 the ARD.  They think I am crazy. We give them the Annual Assessment list well in advance.At least 2 1/2 months Please let me know what you do in your facility...   Thanks
