Therapy screens with the ARD for our residents and this is done on a quarterly basis.  Remember just because therapy picks someone up does not necessarily mean that you have to do a significant change.  I will carefully assess the criteria and would try to obtain from my nursing assistants if the resident has had an ADL decline rather than depending on the therapists to give me this information.  Also, you need to assess what the resident is doing across the 3 shifts and not just for therapy.   Brenda W.
