In the August 2003 Changes To The 2002 RAI User's Manual it says "Page 3-159 clarifies the definition of an ulcer. It deletes the reference to open lesions/sores due to injury and adds that skin tears/shears are to be coded under other skin problems or lesions present (item M4) unless they have developed due to pressure. This page also states that a scab should be considered a Stage II ulcer."

So, is the skin tear that's turned into a scab now a Stage II even though it wasn't originally caused by pressure?

Thanks,

Wounds are staged for M1 only if they meet the definition for coding in M1.  If the wound does not meet the definition for coding in M1, then it still doesn't meet that definition when it is scabbed.

I was just wondering how  your skin team are coding skin tears as the heal related to injury.

Currently, where I work they are coding skin tear initially but as the healing process continues the following week upon assessment their writing scabs. If this is correct then, you have to code it in section M1, and if so, does that effect the quality measures and / or quality indicators for the facility. Responses deeply appreciated.

  once again thanks as always
