ROM and balance

We are in the process of revising our current data collection forms to obtain the MDS info for ROM and Balance. Rather than starting a new form from scratch, was wondering if anyone had one in use that includes the specific directions per the RAI manual or an accompanying policy and procedure that covers the specifics of obtaining this information.
