When I review the QI's I look at each QI and then check the MDS to see if it was an error due to miscoding or an actual incident.  QI #17 decline in late loss ADL's, I check the current MDS and the previous one to determine if it was coded wrong.  In this instance the prior MDS made the resident more independent than the current one.  When I completed the most recent MDS I should have noticed the decline in functioning, but I did not.  I hope this makes this as clear as mud.
