We had a pharmacy inservice this week, and the 6th
Right that they mentioned was "the Right Documentation."
It seems to me that the whole issue of
documentation, whether referring to holes in the MAR/TAR, or inadequate
narrative notes, is becoming more and more critical. When I was in nursing
school, almost 20 years ago, it was absolutely drilled into our heads that "If
it wasn't documented, it wasn't done." I wonder now, if that is being
taught the same way. I just sat in on a counseling and written warning for
a nurse who changed a comfeel dressing every other day for several weeks,
and NEVER ONCE wrote anything about the wound in her nurses notes. Her
defense: "That's what we have a wound care nurse for. I don't know
the difference between a stage 2 and a stage 4"
So, I am all for making documentation the 6th
