so you must be care planning your little head off. We have done the documentation thing for as long as they are, say on an antibiotic or therapy, (10 days). If it makes it to the MDS, (ok if it is near the quarter).
Do you have preprinted episodic Care plans?

We do an acute plan of care and usually chart for 3 days at least q shift on the "apoc". Then it goes in the chart and MDS decides if we need a problem on the Care Plan. Thie system has saved us in the area of documentation and Care Plannning

We do a handwritten short term care plan, putting it on the calendar when to do follow up on the care plan.
