I agree with the below. The intent of the question is to identify risks & problems-yes? This can always be explained in nurse, RD note or right on the MDS. The situation may be a problem for different reasons ie non compliance with therapeutic diet.

We have several residents who literally never eat the meals we serve.
They get take out delivered or their families bring them food from home.
There are no weight issues with any of them, unless on the over weight
side. Our RD thinks we should be coding K4c on the MDS because they
leave 100% of the meals that we serve on their tray. I've never run into
this before. What do you all think?
