The Manual defines
physical restraints (3-198) as "any manual method or physical or mechanical
device, material, or equipment attached or adjacent to the resident's body that
the individual cannot remove easily which restricts freedom of movement or
normal access to one's body." It goes on to describe the process.
Have you walked yourself through this process and asked yourself the questions
posed here? "The assessor should not focus on the intent or reason behind
the use of the device, but on the effect the device has on the
resident."
Also, please see the
bottom of page 201 "Bed Rails Used as Positioning Devices." From your
description, it doesn't sound like she can easily remove the
rails.
