The manual specifically states that ROM assessment does NOT need to be done by a physical therapist.  Page 3-107 "This is a screening item used to determine the need for a more intensive evaluation.  It does not need to be perfromed by a physical therapist.  Rather, it can be administered by a member of any clinical discipline in accordance with these instructions."
I don't think that any specific form is required.  We just use the MDS form to record answers when completing the ROM assessment (done by our nurses).

I think I asked this question once and did not receive an answer.  We were cited for inaccuracy on G-4.  I have a Briggs ROM Assessment form..my question is, does nursing anywhere do this, or is it always therapist, and does anyone know if it has to be a therapist?  If our PT inservices us can nursing do it? thanks.
