Medicare/Medicaid

I have a question and I cant believe I forgot this...I think I'm having TIA's!!!!   Pt is MA  goes out has qualifying stay--comes back medicare  therapy does NOT pick her up  stays medicare for 21 days  NOW  5 day was done and a 14 day....How does she go back to MA????  Do I have to do a MA change???    Thanks for the help!!!!
