Most likely your MI is coming from residents with Depression. Check it out.

The instructions for form 802 are on the CMS website. Here is the link to that part of the site. It is a .pdf file so you need Adobe to read it. It lists what parts of the MDS triggers answers for the form and which sections need to be figured out manually. Many of the sections do not agree with other reports and parts of the MDS so that proves it is a government form! They also do not coincide with the 672 form, either. Good luck.

Subject: HCFA 802 form

I have a question about what items on the MDS relates to the resident characteristics on the 802?  I have looked through the manual and cannot find anything.  We are due for survey anytime and I like to get things together in case they come in off hours.  I was reviewing the 802 and noticed that several resident's had MI checked and they do not have a MI diagnosis.  Any help will be greatly appreciated.
